[docs] Added HACKING.xml file
Moved and improved the coding conventions on its own chapter (HACKING)
and added what needed to properly include this file in the ADG
documentation. Refurbished CONTRIBUTING.xml by adding the "Spread"
section and linking CONTRIBUTING.
