   5 <h4>Entering a referral or forms information</h4>
   6 After filling out a form for a patient or making a referral appointment, from his or her summary screen, click on transaction.<br>
   7 Choose the type of transaction from the drop down menu. <br>
   8 Type details about the form and what we filled out in the input box.<br>
   9 Then click on add new transaction. <P>
