ref updates: include all current heads in ref-changes message
The recipient can use these extra values to reconstruct the state
of all refs/heads just prior to the first ref change that's part
of the ref-changes message.
And this reconstructed refs/heads state can then be used to
produce more accurate ref update notifications.
