xapian-check-patch: Exit with status 1 if errors
Lack of exit code status reported by James Aylett.
Distinguish "error"/"warning"/"note" - currently everything is an
"error" except:
* Removing a FIXME is a "note" (it's a good thing!)
* Adding a FIXME is a "warning" (it's not good, but we don't want
people to avoid adding "FIXME" comments to placate the script
Also don't report FIXME issues in ChangeLog files - "Removed FIXME"
is fine in a ChangeLog entry.
