Set SKI/AKI when creating certificates (based on RSA public keys). Test for
this. Remove the pkcs12 password member variable from wvx509, and make it
an argument passed to the pkcs12 methods. Add a test for pkcs12 encoding/decoding.
Add method to encode a certificate element to a buffer (needed when we want to
write a DER-encoded certificate). Remove "DER64" encoding type for certificate
encoding/decoding: it's really quite useless.
