Rename ffmpeg-mt submodule to libav, delete non-mt/mt logic
Rename the "ffmpeg-mt" submodule, which already contained Libav code,
to the name "libav". Remove various code related to the earlier
support for alternative FFmpeg and FFmpeg-mt submodules and switching
between them.
