Use the actual width of the accented box when building extensible accent
Previously it took delimiter_shortfall and delimiter_factor into account
but it does not confirm with the behaviour of MS implementation, nor
plain TeX's hackish \overbrace nor non-extensible accents. It seems to
have been used to workaround brokenness of get_delim_box().
