More dynamic Makefile & converted manpage to mdoc.
DONE:
* The ii manpage is now formatted using mandoc.
* The documentation has moved to the doc/ directory.
* Fixed the formatting in CHANGES.
* The Makefile is rewritten in order to be more dynamic and flexible.
TODO:
* Go through the documentation.
* Fix the /t command in ii.c (see manpage).
