Fix crashing bugs

We have been plagued by crashing bugs lately and I have been hard at
work trying to squash these.  With the help of several users on this
list I have managed to track these down finally and am releasing this
snapshot in the hope that it will be a lot more stable than snapshots 30
and 31.  I rely on the fact that the readers of this list will try out
new snapshots and test them for me since I cannot possibly do all the
testing myself.  Please, put the new snapshot through its paces and let
me know how it works.

Changes since snapshot 31:

- Console dialogs work once again (set go+=c)
- Background processes work (:sh ... &)
- Ctrl-Shift can be used for modeless selection
- No crashes with HTML plugin
- Toolbar is a bit better at finding icons (this still needs some work)
