Added a new billing widget and following related changes:
1) Added Billing Widget to top left of patient summary.
2) Billing widget includes amount owed, billing note, and
Tony's vital insurance stuff.
3) To improve patient summary changed the fixed widgets that
don't use tabs to not have a fixed height.
4) Since billing widget now contains the vital billing information,
removed this information from top of summary screen and from the
pnotes widget.
