Make kartero the primary, add missing files to MANIFEST, docs and fix
- Make the kartero script as the primary, instead of the
Net::SMTP::Server::SmartHost module.
- Add missing files (test scripts and modules) to MANIFEST.
- Add some initial documentation to kartero script.
- Fix a type in AuthClient (8BITMIME, but it shouldn't really matter...)
