some small focus fixes... if someone sets focus to an override
redirect window *without* grabbing the mouse, then we should react to
the focus out even as if it was a normal focus out event (instead of
ignoring it).
this makes rdesktop sort of work in full-screen mode (since they do
this kind of evil thing)...
